mechanisms, and torsion bar assemblies to include U-bolts, shackles, center bolts, and
hangers. These parts must not be broken, and they must be properly adjusted and
lubricated, where appropriate.
Item 17. Inspect all the tires for cuts, bruises, breaks, and blisters. (A cut extending into the cord
body would be unacceptable.) Check to make sure they are properly matched on dual-
wheel-equipped tractors and trailers. Make sure there are no stones stuck between dual
wheels.
Item 18. Check the cargo space to make sure that there is no oil or grease on the floor and no holes;
exposed bolts, nuts, screws, or nails; or projecting parts.
Item 19. Inspect the electrical wiring to make sure the insulation is not frayed. Check for improper
splices or connections.
Item 20. Check the tail gate and doors to make sure the hinges are tightly fastened to the body and
that there are no broken latches or safety chains.
Item 21. If the vehicle has an open bed and needs a cover, inspect its fire- and water-resistant
tarpaulin to make sure it is in good condition. Generally, on-vehicle-material (OVM) military
tarpaulins in good condition are fire and water resistant. A tarpaulin is not required if the
load is to be in a certified fire- and water-resistant container. For a commercial vehicle,
contact the local fire department for advice on how to determine if a tarpaulin is fire and
water resistant.
Item 22. This block is used only if defects not covered in the first 21 items are found during the
inspection.
Approval or Rejection
If any item is found to be unsatisfactory during the inspection and is not corrected by the carrier, the
inspector must reject the vehicle by making an X in the Rejected box at the end of the second part of
the DD Form 626. If the vehicle passes every item in the inspection, or has its deficiencies corrected
by the carrier (as was the case in the inspection recorded in figure 1-1), the inspector makes an X in the
Approved box and signs his or her name in the Signature (of Inspector) block. Distribution of the DD
Form 626 is not made until after the loaded vehicle is inspected.
In a case where a commercial vehicle is rejected, a copy of the DD Form 626 must be sent to each of
the following:
The nearest field office of the US Department of Transportation.
