Item 17. The tires must also be in good condition. Examine them for cuts, breaks, and blisters.
Check to make sure the tires are properly matched on dual-wheel-equipped tractors and
trailers. Check to make sure there are no stones stuck between dual wheels.
Item 19. Electrical wiring must be inspected to make sure the insulation is not frayed. Check for
proper splices and connections.
Item 20. Tailgates on cargo vehicles and doors on vans and trailers must be secure. Check for loose
or broken latches, hinges, and safety chains.
Item 21. Inspect the tarpaulin. It must be fire and water resistant. (The tarpaulin that is part of the
OVM on military vehicles is fire and water resistant.)
If all items marked with an asterisk are satisfactory, sign the DD Form 626 in the Signature (of
Inspector) Destination block. Continue inspecting through to the end of the form. If you find the items
in good condition, print "SAT" in the Destination column. If you find an item not in good condition, print
"UNSAT" in the Destination column. Proceed as follows:
Item 24. Inspect the blocking and staying. If it is not correct and it is hazardous to move, the vehicle
must be off-loaded.
Item 26. If it is an open vehicle, it must have a fire- and water-resistant tarpaulin. If it is a closed
trailer or van, the seals must be present and serviceable.
Item 29. Check the placards. They must be present and be the correct ones for the load. This
information can be found on the DD Form 836 and DD Form 1384 that you got from the
driver.
Item 30. If the shipment has not been made under the provisions of DOT Special Permit Number
868, enter NA in the Destination column.
Approval or Rejection
If the loaded vehicle is satisfactory, sign your name in the Signature (of Inspector) Destination block at
the bottom of the form, and have the driver sign in the Signature (of Driver) Destination block. If the
loaded vehicle is not satisfactory, disapprove it and enter in the Remarks section on the back of the
form your reasons for doing so. If the loaded vehicle is disapproved, it must be off-loaded, and the
items must be loaded onto an approved vehicle.
Deliver the signed DD Form 626, DD Form 836, and the shipping documents (DD Form 1384 and DD
Form 1348-1) to the control section.
PREPARING DD FORM 836
To fill out DD Form 836 (Special Instructions for Motor Vehicle Drivers) (figure 3-6), you need a DD
Form 626 (Motor Vehicle Inspection) with its top part completed (see figure 3-1). You need the name of
