In case of accident-whether or not the Additional notification required block is filled out depends on
local policy. If additional notification is required, enter that information. If it is not, print NA in the
block.
Other specific precautions or instructions-any special precautions for the items being shipped given
in AR 55-355. See the note on the extract in figure 3-9, for example.
Signature of shipper representative-the place the inspector signs after all the necessary information
has been entered on the form.
Signature of first driver-the place where the driver signs.
The completed DD Form 836 is attached to the partially-completed DD Form 626, and both forms are
given to the driver of the vehicle.
