c. DD Form 1348-1, Automated, Single Line Item Release/Receipt Document (Figure 4) is used to provide
advanced notice of pending ammunition shipments. It may be used in lieu of, or in conjunction with the
DA Form 5210-R (XBT Record) (SAAS Network).
d. DD Form 1384, Transportation Control and Movement Document (TCMD). See Figure 5, page 12. This
document is used to request transportation, plan the movement, and nominate the carrier.

e. DA Form 1298, Due Out Record. See Figure 6, page 12. This document is used when you receive
notice of shipment from your storage area. You will obligate the shipment using this form.
f. DD Form 1387, Military Shipping Label (Figure 7, page 13). DA Form 1387-1, Military Shipping Tag
(Figure 8, page 13). DA Form 1387-2, Special Handling Data/Certification (Figure 9, page 13). These
items are used to mark the materiel being shipped.
g. DA Form 3151-R, Ammunition Stores Slip (Figure 10, page 14). This document is used to inspect, review
and verify the condition of lots selected for shipment. It is also used as an internal control document by
storage facility personnel.
h. DA Form 5210-R, XBT Record (Figure 11, page 14). This document is used as an electronic means of
transmitting advance notice of the pending ammunition shipment, using the standard Army ammunition
system (SAAS) network.
i. Standard Form 361, Transportation Discrepancy Report (Figure 12, page 15). This document is used to
report damage or improper shipments due to transportation discrepancies. With serious damage or
unusual circumstances, the form should be supported with photographs.
