t. Block 12 - By. The unit commander or his designated officer will sign this block and enter the
Julian date.
4. The unit now forwards the DA Form 581 through the DAO or other designated officer to sign Block
7a. Then the DA Form 581 is sent or handcarried to the SSA/ASP for processing.
5. At the SSA/ASP, the DA Form 581 will be checked to insure that the persons who have signed Block
7a and Block 12 are authorized to do so. To accomplish this, each SSA/ASP has a DA Form 1687 on
hand for each using unit authorized to draw ammunition from that SSA/ASP. The SSA/ASP also has a
second DA Form 1687 from the DAO which authorizes personnel to sign Block 7a of the DA Form 581.
Preparation instructions for the DA Form 1687 (Notice of Delegation of Authority Receipt for Supplies)
are contained in DA Pam 710-2-1 and DA Pam 710-2-2. See Figure 2 as an example of a DA Form
1687.
