Authenticating officer signs in this block. Depending upon the type of

Enter the office symbol of the authenticating office.

Enter an "X' in the box designating a "Transportation Order". Enter
"Training Ammunition" in the space below the transportation order block.
k. Block 10a
Enter the item numbers, in sequence, for each item requested.
1. Block 10b
Enter the Federal Supply Class (FSC), Department of Defense
Identification Code (DODIC), and description of each item requested.
Enter the words "Last Item" after last entry.
m. Block 10c
Leave this column blank.
n. Block 10d
Enter the quantity of each item requested.
o. Block 10e
Leave this column blank.
p. Block 10f
Leave this column blank.
q. Block 10g
Leave this column blank.
r. Block 11
Enter one or more of the following statements, as applicable:
(1) Quantities requested are within training authorization.
(2) Pick-up of ammunition is required on (enter date)
(3) Applicable training event code is (enter training event code from Appendix A of this
subcourse).
s. Block 12
The unit commander or unit ammunition officer will sign and enter the
Julian Date.
t. Block 13
After the ammunition has been issued by the ASP, the authorized unit
representative will sign this block and enter the Julian Date.
