The SOP Supervisor's Statement will be placed directly beneath the cover sheet and should have a format similar
to that shown in Figure 1-3. The supervisor having the closest supervisory authority over a particular operation
will sign for that operation when the following events occur:
1. When he is first assigned as supervisor of the operation.
2. When an approved formal or interim change is made to the SOP.
3. When an operation is started up after 6 months in an inactive status.
4. At least once a year.
The next element to be prepared is the Operator's statement (See Figure 1-4).
