NOTE:
When returned to the SSA/ASP, serviceable ammunition whose lot identity has not been lost,
will be added back into the unit's training authorization. DA Form 581 must be submitted to
the ASP for turn-in.
(9) After the issue or serviceable turn-in has been completed, DPT/DOTSEC will be provided a
copy of completed transaction (DA Form 581) by the ASP. Check the form against their approval
register, and retain it for data terminal/bulk submission to DA, PODCSOPS. DPT/DPTSEC personnel
responsible for operation of the teleprocessing terminal will process (not less than twice weekly),
transaction data reflected on DA Form 581 as outlined in the TAMIS Terminal User Instruction Booklet
provided by HQDA. One copy of each DA Form 581 will be bulk mailed to DA weekly, and more
frequently if volume dictates. Forms will be annotated to reflect terminal processing completion. The
last transaction, DA Form 581, for each month will be forwarded by the fifth working day of the
succeeding month, and annotated to show that it is the last transaction form and listing the total lines
forwarded for that month. Copies of DA Form 581 will be mailed to HQDA, ATTN: DAMO-TRS-
TAMIS, Wash DC 20310. Copies of DA Form 581 will be retained on file at each installation for at
least three months after the transaction month.
