INTRODUCTION
In order to be successful in any war, the required amount of ammunition must get to the using units in a
serviceable condition. In this lesson, the procedures required to issue ammunition to the using unit and how
ASPs determine which ammunition is issued at a particular time will be discussed.
As it applies to the issue of ammunition, the term "issue" refers to the transfer of ammunition stocks from an
ammunition storage facility to an authorized user of ammunition. Remember, the transfer of ammunition stocks
from one ammunition storage facility to another storage facility is referred to as a shipment and was covered in
Lesson 1.
REQUESTING AN ISSUE
Prepare DA Form 581
When ammunition is required, the user prepares a DA Form 581 (Request for Issue and Turn-In of Ammunition).
See Figure 2-1. This form is used in peacetime as well as in wartime, although processing procedures change
somewhat.
The requester at the using unit prepares the DA Form 581 as follows:

Places an "X" in this block.
Block 2-Turn-In
Leaves this block blank.
Block 3-Document No.
Enters the unit document number from the expendable document register. The unit document number consists of
the unit DOD activity address code (DODAAC), the Julian date, and the serial number.

Enters the total number of pages associated with this request-DA Forms 581 and DA Forms 581-1 (Request for
Issue and Turn-In of Ammunition Continuation Sheet).

Enters the name, the address, and the unit identification code (UIC) of the using unit.
