person to authenticate ammunition requests (sign Block 14c of DA Form 581).
Using-unit commanders must designate, on a DA Form 1687 submitted to the SAAS-4 activity, unit members
authorized to request and receive Class V items (sign Block 13c and Block 31b of DA Form 581). See Figure 2-2.
The using unit prepares a DA Form 1687 to authorize personnel to request and receive Class V ammunition as
follows:

Enters the calendar date the form is prepared.
Organization Receiving Supplies
Enters the name of the requesting unit.
Location
